Fordham University Military Hall of Fame Nomination Form

Please fill out the form carefully and completely. It is the only document seen by the selection board members.

No attachments or additions are allowed, except a copy of the citation for a significant award such as the Bronze Star or higher, or a military biography. Attachments of this nature may be emailed separately to the selection board at veteransalumnichapter@fordham.edu.

The military service section may be in narrative form; however, it is easier for the board to read if it is in outline form, listing first the military service and ranks and duties in chronological order, then awards and significant achievements.

The 250-word biography should be based on the most significant parts of the individual’s service. The word count will be checked by Microsoft Word. The nomination may be returned without action if the word count is exceeded. This biography will appear verbatim in the Hall of Fame program.
